  Electoral Office Services and Forms

On this page you will find a range of forms commonly asked for at the electoral office. Some of these forms require copies of drivers licenses or passports to be sent with them for identification purposes. This needs to be witnessed by a Justice of the Peace (Commissioner of Declarations or Qualified). Both of my staff in the office are C Decs and can certify your identification as well as witness signatures for other documentation. If required, please make sure to bring photocopies of the identification you are using so they may be certified.

Please select the state you were born in for the relevant certificate, a small fee is required -

JP forms:-

If you wish to become at Justice of the Peace (C Dec or Qualified) the forms are listed below. Please keep in mind you need to have the declaration witnessed by a current JP, two referees who have known you more than 5 years and if you are applying for the JP Qualified, you must have a copy of the letter saying you have passed the exam. Once you have those forms in order, bring it into the office so Peter may sign off on it.

Celebratory Letter :-


Celebratory letters can be requested for a number of different wedding anniversaries and birthday milestones. Along with the completed form, the original marriage or birth certificate must be presented. If the certificate cannot be presented, a statutory declaration by a family member can be used in its place.

Enrolment Form:-

In Queensland voting is compulsory for Federal, State and Local Government elections. If you are eligible to enrol, you must or risk the chance of a fine. If you move house you must submit a new form with your details.

Seniors Card:-


Are you 60 or older? If so you are eligible for the Seniors Business Discount Card. This scheme operates through partnerships with businesses and Government service providers by providing business discounts and Queensland Government Concessions on service charges. Queensland Ombudsman Form:-

The Queensland Ombudsman is an independent body responsible for investigating complaints on public agencies. Their role is to make sure Government acts fairly and makes he right decisions for Queenslanders. The Ombudsman covers a wide range of areas including legal, electricity and health. If you believe a public agency has made an incorrect decisions about an issue which affects you, fill out a form below and mail it to the Ombudsman.
